Performance of multiprofessional home care teams with a focus on follow-up on dependent elderly people

This study aims to analyze the scientific production about the performance of multiprofessional home care teams with a focus on monitoring dependent elderly people. An integrative literature review was carried out by consulting the MEDLINE, LILACS, IBECS and BDENF databases. Articles in Portuguese, Spanish and English published between 2015 and 2019 were included. The basic search strategy was: “Patient Care Team” OR “Nursing” AND "Home Care Services" AND “Elderly”. Of the total number of articles found, 37 answered the guiding question and, therefore, constituted the final sample of this review. Two categories emerged, which are described and discussed, namely: “Multiprofessional care for the elderly” and “Care centered on the elderly”. This study shows us the importance of multiprofessional teamwork for care centered on the person and their family members, as well as providing care in terms of encouraging the preservation of autonomy and independence, as well as strategies for facing central issues in care at home, such as shared decision-making, communication, family dysfunctions, overload, dependence, pain and palliative care, medication use, falls, pressure injuries and mental health.
